Early Childhood Screening

All 3 to 5 year old children in Minnesota are required to complete early childhood screening, including a hearing and vision check, speech and developmental screening, and health and immunization screening.

In addition to the completed forms, parents need to bring in the following:

  • Child's Birth Certificate (A copy is acceptable)
  • Child's Immunization Record

Screening is required before entrance into kindergarten but is not a screening tool for kindergarten. It is offered by White Bear Lake schools at no cost. Screening at age 3 is preferred, but a child may be screened between the ages of 3 and the start of kindergarten as required by law. Screenings are held throughout the school year and normally take 1 to 1.5 hours to complete. Click here for a quick video. 

As part of the process there are several forms parents are required to complete. One form is electronic and will be done at the screening appointment. Parents are encouraged to fill out the paper forms prior to the appointment. Click on the links below to access the forms:
